Get To Know Dr. Zak Taylor
Dr. Zak Taylor has been serving the Mayfield and Murray communities for many years. He began his career in 2014 by opening his first practice, Mayfield Family Dentistry, which quickly became a trusted name in the area. In 2018, Dr. Taylor expanded his practice by purchasing the office of Dr. Steven Farmer in Murray, which has since been rebranded as Elevation Dentistry. With years of experience, a passion for dentistry, and a commitment to providing his patients with the highest level of care, Dr. Taylor has become a well-respected member of the dental community in Kentucky.

Dr. Orr is not only a proud Murray native, but also a highly skilled and compassionate dentist with a passion for serving his community. With a degree from the University of Kentucky College of Dentistry and years of experience in the field, Dr. Orr brings a wealth of knowledge and expertise to his practice. He is dedicated to providing the highest quality dental care to his patients and is committed to staying up-to-date with the latest advancements in dentistry. As a Murray High School graduate, he has a deep understanding of the needs and values of his hometown and is honored to be able to give back to the community that has given him so much.

A Seamless Experience
Your First Visit
As you walk into our office, you’ll be greeted by our warm and welcoming team and the comforting amenities of our family-friendly office. We’ll answer any insurance or financing questions you may have to make your appointment as seamless as possible.
You’ll meet with Dr. Zak Taylor or Dr. Stephen Orr and receive a comprehensive oral exam, complete with any necessary x-rays. Then you’ll meet with one of our hygienists for a thorough cleaning. We always spend the extra time to address your concerns and discuss the state of your dental health with you before outlining a unique treatment plan tailored to your needs. Before leaving, we can also schedule any future visits and sign you up for our annual membership plan.
What to Bring
To make sure your first visit goes as smoothly and as quickly as possible, be sure to bring the following items to your appointment:
- Photo ID
- Insurance Card (if using) and/or Form of Payment
- Completed New Patient Forms
- Questions

We are committed to being an integral part of the city we call home and building our community of Mayfield. Mayfield Family Dentistry donates to local groups and charities, and all local athletic programs. Dr. Taylor is also on the board of the J.U. Kevil Foundation that focuses on providing services and programs for those with mental, physical, and/or developmental disabilities.
